Little did the MLB know that behind the scenes, Bonds and other athletes had been using a newly synthesized steroid, tetrahydrogestrinone (THG), designed specifically for potent anabolic effects and with anti-doping testing protocols in mind. Dubbed “The Clear”, THG could not be detected in urine initially because the anti-doping program had no knowledge of its existence or metabolites. During an investigation, a sample of THG was extracted from the residue of a spent syringe and identified, after which an LC-MS/MS method could be easily developed for screening4. Gene doping refers to the use of nucleic acid sequences (delivered either as naked DNA or through viral vectors) and/or normal or genetically modified cells to enhance sports performance (385, 404, 405). Gene doping has not been detected in any sports event to date, although many experts have predicted that gene doping will become a reality in the near future (385, 404–408).

Many sports will not allow the presence of Beta Blockers at any level, despite an athlete’s claims that it may have been used for a legitimate medical reason. When used for a legitimate medical reason, for example to treat a heart condition, high blood pressure anxiety, and under the guidance of a trained professional, beta blockers have a good safety record. Researchers are working on a detection method that will look for the effects of EPO in the body’s cellular anatomy as opposed to its presence in urine or blood. In essence the new test will look for genetic expressions which prove that EPO has been used. If the new test proves accurate and viable, it will be almost impossible to use EPO without detection.

How do anabolic steroids work?

The detection of synthetic anabolic steroids by gas chromatography/mass spectrometry began in the mid 1980s (361–363). Use of either magnetic sector or orbitrap mass spectrometers in the high mass resolution mode significantly decreased limits of detection and lengthened the detection window (364). Some of the adverse effects seen in patients who use AASs may include infertility, gynecomastia, sexual dysfunction, hair loss, acne, muscular appearance, and testicular atrophy. Some indicators that might suggest AAS use are increased hemoglobin and hematocrit; suppressed LH, FSH, and testosterone levels; low high-density lipoprotein cholesterol, and low sperm density. Mass spectrometry-based tests (available in many commercial laboratories) can detect AASs in urine. Testosterone abuse is more difficult to detect, but high testosterone, in association with suppressed LH and FSH levels, should raise suspicion of testosterone abuse.
